All registered campers and camp families will receive, via mail, a Camper Packet Catalog. In this catalog, you will find information and instructions regarding the online completion of registration forms via your My Ozark account. If you are unable to complete your registration forms online, or if you prefer to have paper forms, please contact the Camp Ozark office for assistance.

This page contains links to many important forms. Listed here are "Contract Packet Forms" and "Information Packet Forms" (when available). "Contract Packet Forms" are the "nuts and bolts" of your employment, and contains such items as "Personnel Policies", "Camp Ozark Statement of Spiritual Philosophy", and the all-important "Health Card."

"Information Packet Forms" provide you with important information regarding your employment at Camp Ozark, and allow you to provide us with pertinent information as well. Such forms include "Directions to Camp Ozark", "Special Time-Off Request Form", and "Talent Search Form."

Keep in mind that while some of these forms are provided for your information only, others must be filled out and returned to Camp Ozark in a timely fashion.
