For those campers and camp families currently enrolled at Camp Ozark, we are making these forms available in case you need to replace a lost or damaged form. Forms available include Camper Packet forms (Travel Forms, Health Forms, the Camp Ozark Code of Conduct, Policy Agreement forms, an Ozark For All flyer, Store Order forms, Fee Summary form and a form regarding the selection of Morning Activities.) Also available is a list of Hotels and other Accommodations, and Directions to Camp Ozark. Parents may also download a camper application form for either the current year or for the next year.

All registered campers and camp families will receive, via mail, a Camper Packet of information and forms. Many of these forms will need to be completed and returned to Camp Ozark. For those campers and camp families currently enrolled at Camp Ozark, we are making these forms available in case you need to replace a lost or damaged form. This page contains links to many important forms. Listed here are "Contract Packet Forms" and "Information Packet Forms" (when available). "Contract Packet Forms" are the "nuts and bolts" of your employment, and contains such items as "Personnel Policies", "Camp Ozark Statement of Spiritual Philosophy", and the all-important "Health Card."

"Information Packet Forms" provide you with important information regarding your employment at Camp Ozark, and allow you to provide us with pertinent information as well. Such forms include "Directions to Camp Ozark", "Special Time-Off Request Form", and "Talent Search Form."

Keep in mind that while some of these forms are provided for your information only, others must be filled out and returned to Camp Ozark in a timely fashion.
